The "Vial of Life" is a small packet that contains a form that lists you or your loved
one's relevant personal medical history information including any current health problems,
medications being taken, a photostat of you or your loved one's last EKG, a living will
(if one exists), and any "do not resuscitate" or DNR documentation (if you so choose).
The "Vial" is then stored on your refrigerator and marked with the "Vial of Life" sticker.
Another sticker is placed on your front door so that emergency medical personnel are aware
that the "Vial" exists and they can retrieve it for complete medical information.

| Employees Caregivers - NOT Independent Contractors |

There is a tremendous difference between employees and independent contractors.
Companies that use employees pay all payroll taxes, workers comp, etc and are
insured and bonded by the company. Independent contractors are not covered
by Workers Compensation, tax liabilities and cannot be required to follow an
agency set schedule (your schedule) while performing tasks a specific way.
Federal law makes this distinction very clear. For this purpose, it is in
your best interest and ours that ALL caregivers have a status of "employee"
and not "independent contractor".

workers comp, etc. And in the event of an accident, insurance and bond have been paid
by Adaptable Home Care.

About the Society Of Senior Advisors
CSA educates professionals to work more effectively with their senior clients.
We believe that the right kind of planning, recommendations and referrals can make
aging a state to be savored instead of a fate to feared. For those who work with seniors,
that means understanding the key health, social and financial factors that are important
to seniors-and how these factors work together. CSAs are able to integrate this into their
professional practices, no matter what field they're in. They've learned how incredibly
gratifying it is to help seniors achieve their goals, and the seniors they've worked with
have learned how important it is to work with someone who truly understands their age-related circumstances.
What is a Certified Senior Advisor (CSA)?
A Certified Senior Advisor is a professional who has received specialized training in
aging and the important issues that effect seniors. CSA candidates must pass a challenging
national exam to demonstrate their comprehension of what is taught in the CSA curriculum.
It is only after they pass the exam, meet all the requirements, and agree to live up to the
high ethical and professional standards
that they may use the designation Certified Senior Advisor. When you work with a professional
who has added the CSA designation to his or her credentials, you know you're working with
someone who has invested time and effort in learning about the things that are important to you.

Standard 1: Competence. A CSA designee has an obligation to keep his/her
professional training, skills, and knowledge current and comprehensive in order
to competently provide professional services to clients. Such competency must
be maintained and enhanced over time as a result of changes in legal, regulatory,
and professional standards and processes. CSA's who sell products must meet the
same standards with respect to product knowledge and suitability.
Most CSA's hold one or more professional registrations, licenses, or credentials.
All CSA designees must comply with the rules and regulations that govern the
registrations, licenses and/or credentials they hold. CSA designees are often
affiliated with companies and/or employers that have their own rules for ethical
business conduct; CSA's have an obligation to comply with these rules as well.
Disciplinary action taken against a CSA designee by any regulatory or
administrative body can carry the same weight as violations of this Code,
and therefore may result in suspension or revocation of the CSA designation.
A CSA designee who performs services, or offers to perform services,
that are beyond the scope of his or her professional license and/or credential
may be subject to Board action. The CSA credential is a broad-based, value-added
designation that enhances the registrations, licenses and credentials a
professional already holds. The credential bestows on the individual who earns
it a deeper understanding of seniors and the issues important to them,
but it is neither designed nor intended to be a substitute for professional credentials.

| Standard 2: Honesty. A CSA designee shall not lie, cheat, or steal.
A CSA designee must accurately and completely communicate to clients
and potential clients, both orally and in business materials, his or her
professional licenses, credentials, and other business qualifications. Further,
a CSA designee shall not omit from business materials or discussions - or otherwise
disguise in technical, legal, or industry specific terminology ("jargon") - information
that the CSA designee's client or potential client would need to fully comprehend proposed
products or services.

| Standard 3: Trustworthiness. A CSA designee shall act
in a trustworthy manner. This means that the CSA designee shall promptly and courteously
address a client's questions and concerns; shall perform professional services to
achieve the best interests of the client; and shall be dependable, honoring personal
and professional promises made.

| Standard 4: Fairness. A CSA designee shall conduct his or her professional
activities fairly and impartially, using the same standard of care in performing
business activities. Further, the CSA designee shall disclose all potential or
perceived conflicts of interest that may arise in connection with providing
professional services to clients, prospects, and business associates.
A CSA designee shall safeguard client, potential client, and business associate
information, maintaining complete confidentiality unless such party has specifically
released the CSA from such duty, or in the event that such information is required
to be divulged in response to proper legal mandate.

serve the public, clients or patients, potential clients or patients,
business associates, and employers with the highest degree of
professionalism, exercising due diligence and acting in good
faith in all matters; exhibiting appropriate demeanor and behavior
in all business settings and transactions; maintaining good customer
relationships with respectful, timely, and accurate service;
following best practices in maintaining the books, records, and
documentation of his or her business or practice; and upholding the
standards of the CSA designation and of any other licenses or
credentials he or she holds

Questions You Should Ask A Potential Home Care Company
What areas do you serve?
We serve most of Summit & Cuyahoga counties but are focused in the area between Akron & Cleveland.
Are you licensed by the State of Ohio?
There are no license requirements for Home Care Agencies in the State of Ohio.
Since there are no state requirements, some small companies decide to cut their
costs by not being bonded, insured, pay into workers comp, etc.
Are you insured and bonded?
Yes, we have professional & general liability insurance through the American Alternative
Insurance Corp Policy# VHHG3053458 - $1M / $3M. We are bonded through the RLI
Insurance Company - $25K.
Can I choose my own caregiver?
Absolutely, we always want you 100% satisfied. If at any time you feel the caregiver
is not meeting your loved one's needs, you can request a change in the caregiver.
We match the caregiver to the client's needs.
Are your caregivers employees or independent contractors?
All of our caregivers are employees. We pay worker's comp so that if they get hurt
on your premises they are covered. If you hire a caregiver directly or use an
agency that uses independent contractors you will pay this insurance.
Do you pay all taxes? And am I liable for any?
Yes, we pay payroll taxes. You will not be liable to pay these like you would if
you hired a caregiver direct.
What happens if you have an issue or concern after business hours?
Home care is a 24/7/365 business. There is someone on call for emergencies 24hrs / day / 365 days / year.
How do you handle customer complaints / caregiver conflicts?
Gregory Cochran, the owner, handles all complaints / conflicts personally.
What type of screening do you perform on your caregivers?
We have one of the most extensive screenings in the industry.
All caregivers go through the following process:
- Phone Interview
- Caregiver Quality Assurance Assessment*
- In-person interview
- Caregiver Exam / Test
- Reference Checks
- Ohio Sex Offenders Database
- National Sex Offenders Database
- National Criminal Database (includes eldercare & child abuse offenses)
- Social Security Trace
- DMV Check
- Drug Screen (and ongoing random testing)
*We use a unique pre-employment assessment system that examines attitudes, behaviors
and personality to screen applicants before an incident can occur!!
This assessment system measures:
- Dependability - Measures the risk that an individual will be undependable,
careless, lazy or disorganized
- Honesty/Integrity - Measures the risk for dishonest behavior.
- Hostility/Aggression - Measures the risk of a person being
aggressive, hostile, and disruptive or having poor control of their anger.
- Substance Abuse - Measures the risk of substantial use of alcohol or illegal drugs.
- Sexual Harassment - Measures behaviors regarding sexuality that are likely to be considered harassment.
- Computer Misuse - Measures the risk of using computers and Internet connections in ways unrelated to job tasks.
- Cognitive Reasoning - Evaluates problem-solving abilities and learning speed.
Not many companies use the Caregiver Quality Assurance™ program because of the added cost.

How often do you change caregivers?
If you have consistent scheduled hours weekly we will make every effort to provide you with the same caregiver(s). We understand that client / caregiver relationship is one of trust so we attempt to provide the same caregiver for all service. However by Ohio law any hours over 40 in a given week will be paid at time and a half. You will have the choice to pay the additional amount or chose to have a new caregiver assigned for those hours.
What type of care do you provide?
- Companion Care includes meal preparation, light housekeeping, assistance with daily activities such as letter writing, reading, and entertainment.
- Personal Care includes the basic Activities of Daily Living (ADLs) - bathing, dressing, toileting, transferring, and feeding.
Do you provide live-in or 24hr care? And what's the difference?
This is sometimes confusing as companies have their own terminology.
Live-in is when a caregiver lives with the client because they cannot be left
alone but doesn't need constant support. Client usually sleeps through the night.
The caregiver typically provides care as needed but regularly gets 8 hrs of
uninterrupted sleep and 8 hrs of "personal" time. Caregiver is there for
emergencies and intermittent care.
24 hour care is when the client requires constant support.
In this instance, several caregivers would support the around the clock schedule.
Will you provide transportation?
Yes, we provide transportation to and from doctor's appointments, hair / nail salons, errands,
family and friends, the grocery store and other places as needed. We carry special insurance for this.
What other services do you provide?
When you're our client, we act as a referral and information service.
We can connect you with Elder-law attorneys, In-Home Salon Services,
Podiatrist, Reverse Mortgage Specialists, Ramp Installation Services,
Medical Equipment Suppliers, Delivered Meals, Lawn Services, Plowing,
Financial Services, Medicaid Planning services, etc... all services that will help you remain in your home.
Do you do free home assessments?
Yes, we never charge for an assessment. This is an opportunity to learn more about the care
recipient's needs as well as for us to get to know each other personally.
Who does the assessment?
Normally, the owner will meet with the patient and their family in the home, hospital,
or rehab center to discuss our services, medical history, and develop a personalized care plan.
Do family members need to be at the assessment?
No, but relationships are very important to our agency and we believe that family participation
helps to build relationships and keeps the lines of communication open. A neighbor or friend
could also attend the meeting if a family member is not available.
What funding is available to help pay for care?
Generally speaking, personal care or companion services are not covered by insurance or Medicare.
If you have purchased long-term care insurance it may cover a portion of your costs for in-home care.
The fastest growing segment of home care is private pay home care, where clients and families
choose to use their personal financial resources to provide the peace of mind, quality of life,
and independence that comes with staying at home rather than moving to a facility.
Do you accept Long Term Care Insurance?
Yes, we accept Long Term Care Insurance. We also submit all paperwork that is required to your
provider. Some examples of providers are: Genworth, John Hancock, MetLife, Prudential,
Blue Cross / Blue Shield, Mutual of Omaha.
Is there anything to help Veterans obtain care?
Yes. Aid and Attendance could apply to Veterans and / or their spouses.

Are you a franchise?
No, Adaptable Home Care is a locally owned and operated business. We are part of a
national network called Companion Connection.
How old must a client be?
Care is provided to for all ages not just the elderly.
What is your dress code?
We ask that our caregivers dress professionally: Mostly clean neatly pressed slacks and a collared
shirt with the Adaptable Home Care logo. They also wear an identification badge.

Is there any long term obligation?
No, there is no long term obligation. The first week, you can cancel for any reason at any time.
After the first week, we ask that you give us a 72 hour notice, a week if possible.
Do you charge Overtime or Extras on Weekends?
No, we charge an hourly rate based on the number of hours that are purchased.
Do you charge Overtime or Extras on Holidays?
Time and a half is charged for holidays.
Who Refers Customers To You?
Several of our premium local Hospitals, Skilled Nursing Facilities, Elder Law Attorneys,
Mortgage Brokers, Senior Centers, Medical Equipment Companies Senior Apartments, etc...
Do you have a minimum number of hours?
No.
